SDM started fundraising in 1986, and raised enough money to help two patients.  Since then, SDM has served over 5000 patients.

Is my donation tax deductible?

Yes. SDM is exempt from federal income taxes under Section 501(c)(3) of the Internal Revenue Code. Donations are tax-deductible to the fullest extent allowable under the law.

Will SDM accept company matches to my donation?

Yes.  Please contact your employer directly for their process to match a donation.  You will need our official name - Susen Daniel Memorial Cancer Relief Fund - Tax ID #33-0111733.  If your company requires more information please contact us so we can assist you.​

 

Can I send my donation in the mail?

Yes. We accept check donations by mail, but please do not send cash. Print this PDF form and mail to:
